Diabetes management services are available on an ongoing basis. A Dietician and Diabetes Educator attend groups and one-to-one sessions monthly in person or by using our Telemedicine Services.

The multidisciplinary professional team provides medical, diet and lifestyle services to help people self-manage chronic disease. If you are suffering with diabetes, heart conditions, emphysema, HIV, FASD or other chronic conditions our experts can help.
